Any students wishing to participate in athletics must have the following turned in to the front office prior to the first day of practice:

• Authorization to Participate Form (current school year)

• Current Sports Physical (less than one year old) signed by a physician

• Current Concussion Form (less than one year old)

• Activities Handbook Acknowledgement Form (must be updated every school year)

• $40 Activity Fee (scholarships available, please contact Dan Ries, Athletic Director, at dan.ries@target.k12.mt.us)
